# Collect leads with a form

> Configure a public form, choose the records it creates, and check the resulting contacts.

A lead form lets visitors send contact details into your workspace. Work must be available, and an administrator must configure the form before you share its public link.

## Build and test the form

1. Open **Settings > Lead forms** and create a form.
2. Choose whether submissions create a person, a company, or either according to the submitted fields.
3. Enable and order the fields you need. At least email or phone must be required. Company-only forms must enable the company name.
4. Select the default owners. Configure notifications and automatic Deal creation if your process needs them.
5. Save, review the preview, and submit a test through the public form. Check the resulting contact, follow-up Task, and Deal when enabled.

Forms support name, company name, CVR, email, phone, and message.

## Understand matching

A company name with a valid CVR can create a company or match an existing one. A company name without CVR can link an existing company but does not create a new one through this form.

People match by email, with a phone lookup for phone-only submissions. Existing contact details are preserved where already filled. A submission can still create new follow-up work for a matched contact, so do not resubmit merely to check whether the first attempt arrived.

## Share or embed

The form detail provides a public link and embed snippets. Copy the snippet from Floral into your website, then test the published page. The preview in Settings does not submit a lead.

Treat the link as public. Request only information your organization intends to collect. Deactivate a form when it should stop accepting submissions. Existing records remain.

## Check the result

The form detail shows company and person counts with links to those records. It does not show a submission-history list. Counts of newly created contacts do not prove whether a submission matched an existing contact.

Open the relevant contact and check its lead-capture activity and follow-up Task. Check assignment before relying on follow-up work. A failed email notification does not undo an accepted submission. If receipt remains unclear, contact support with the form name and approximate submission time instead of submitting again.
